2. Name and address of the office where CLARA-W will be used
3. Mailing address for the shipment
4. Mailing address for the invoice, if different from the above
5. Name, telephone number and e-mail of the person issuing the order
6. Information about any existing licenses of CLARA or CLARA-W in your organization.
